Pre-Patch refers to the game before the Age of the Chosen patch, which significantly altered equipment amongst other things. Usually when something is said to be Pre-patch it is an item that is very statistically different then items that are currently able to be made, as equipment was kept from before the patch. Pre-patch equipment costs very high amounts to repair, however, so usually someone must buy gold in order to keep up with repair bills.
